What to include:

Personal taxable income after deductions (net income)

Sole Proprietorship or Contractors: Enter amount from line 31, Form 1040 on Schedule C

Partnership or S-Corporations: Enter amount from your K-1, adding the income amounts together on lines 1-6

Be precise as we may require proof of income. Income from alimony, child support, and separate maintenance does not need to be disclosed if you do not want it to be part of your application.


Provide the incomes before taxes of everyone contributing to your household other than yourself. If there are no other contributing members, enter zero.
